Please contact the store in advance to make a reservation.

Visit us
Please come to the store on the reserved date and time. It will take about an hour to try on the guest attire.

Try it on
You can see the actual costumes before making your selection. We also have many costumes in sizes and patterns that are not listed in the catalog.

You can apply on the spot. Please ask the staff for details on how to deliver the costume.

Please fill in the attached application form with the necessary information. Please refer to page 16 of the catalog for how to measure your size.

Apply by fax
Please fax the completed application form to the store.

The store will contact you by phone.
We will call you within 3 business days after receiving your fax to confirm your order. After we confirm by phone, your order will be complete. Please ask our staff for details on how to deliver your costume.
